Federal government of Pakistan National Database and registration authority NADRA officially announced that how to apply for B-form and National Identity Card for children below 18 years of age.
NADRA smart identity card or Juvenile card is small identity card that is issued by NADRA to children under age 18 years.
The smart sim ship based card that is also called child registration certificate CRC as it’s an entitled document that give us multiple facilities to the parents of children.
Eligibility to Apply for F-form or ID Card
The baby children having age limit even one day to below 18 years age are eligible to get B-form or smart chip based Smart ID Card.

How to Apply for B-form or ID Card
- The parents or guardian of the children can visit Nadra office during office hours with computerize birth certificate.
- One of the parents or a court-appointed guardian should come along with the computerized birth certificate of the child.
- If one of the parents is present, the application form must be attested by the gazetted officer or public representative (MNA, MPA or local body officials and members).
- Presence of child is required for ID card. Child’s photograph and fingerprints of children above 10 years of age will also be taken
Manual Birth Certificate or School Certificate of Union Council will be acceptable if computerised birth certificate facility is not available in Union Council.
